An Invitation To Participate In Our Employer-provided Long Term Care Program
Congratulations and thank you for providing 15 years of dedicated employment with Perkins Coie. In recognition of your efforts, Perkins Coie Benefits would like to offer you an individual Long Term Care (LTC) insurance policy. This custom LTC program, developed by our insurance partner, Pillar International Insurance Advisors, is offered through National Guardian Life Insurance Company (NGL).
This policy includes:
- Premiums paid by the firm as long as you remain a benefits eligible employee.
- A personal enrollment support specialist to assist you during the process.
- Complete confidentiality in handling of employee data during enrollment.
- Transferability of the discounted premium after you leave Perkins Coie.
- Discounted premium is available to immediate family members of Perkins Coie employees (Grandparents, Parents, Siblings).
- An underwriting process which requires physician-provided medical records, carrier-insured brief telephone interview and possible paramedical exam. No underwriting expenses are borne by the insured.

Perkins Coie Benefits

National Guardian Life Insurance Company’s (NGL) long term care insurance protection product is called Essential LTC. This product is an individual long term care insurance policy that is offered at a discounted rate to Perkins Coie employees. Employees with 15 years of service are eligible for this employer-paid policy.
Plan Details
The Perkins Coie sponsored plan provides $130 of daily benefit for up to two years. The daily benefit will increase with a 3% annually compounding cost of living adjustment. Once you qualify for the LTC benefit, there will be a 90 day waiting period before benefits are paid. If desired, employees can choose to enhance their coverage at the time of application for an additional premium cost borne by the employee.
In order to qualify to receive LTC benefits under the Essential LTC policy, a licensed healthcare practitioner has to certify that within the last 12 months:
You require substantial assistance due to your inability to perform at least two Activities of Daily Living* (ADLs) for a period expected to last at least 90 days due to a loss of functional capacity, OR
You require substantial supervision to protect you from threats of health and safety due to severe cognitive impairment.
*Activities of Daily Living defined in your Policy are: bathing, continence, dressing, eating, toileting and transferring.
Care can be received in a skilled nursing facility, assisted living facility or at home. This policy does provide coverage for Alzheimer’s Disease, Parkinson’s Disease and Senile Dementia.
Enrollment Details
A Pillar representative will enroll you over the phone and will take approximately 20-30 minutes. All application materials are submitted confidentially and electronically directly to National Guardian Life Insurance Company.
National Guardian Life will review your application and will begin the underwriting process by ordering medical records to help determine eligibility. For those applicants age 65 and under, NGL will conduct a brief telephone interview to go over your health history and lifestyle and may include a cognitive exercise as well. For applicants age 66 and over, a face-to-face cognitive assessment will be required.
A Paramed exam (height, weight, blood and urine sample) will be required if an applicant hasn’t seen a health care professional in the last 2 years or may be required at the underwriter’s discretion.
Frequently Asked Questions
How common is the need for long term care insurance? Over 70 percent of people over the age of 65 need long term care during their lifetime. More than 40 percent will spend time in a nursing home (US Administration on Aging 9.21.18).
Where can I receive care? The policy will allow the insured to receive care at home or in a community-based or private facility. Licensed care providers may be chosen by the insured and family.
Why is the policy based on loss of Activities of Daily Living Activities of Daily Living* (ADLs) are commonly accepted by medical practitioners as the instrumental activities of functionality. Loss of two or more ADLs or a cognitive impairment, as determined by your medical practitioner, will initiate benefits.
*Activities of Daily Living defined in your Policy are: bathing, continence, dressing, eating, toileting and transferring.
Doesn’t Medicare pay for long term care services? Medicare Part A covers up to 100 days of care in a nursing facility only after a prolonged hospital stay. Medicare does not pay for custodial care which accounts for 80% of long term care administered (US Administration on Aging 9.21.18).
Will my benefit increase over time beyond $130 per day? The benefit will increase with a 3% compounding cost of living adjustment for the life of the policy.
I would like to increase my initial benefit amount. Can I purchase more coverage beyond the base plan? Yes, benefit levels are available to $300/day with discounted rates. A 5% compound cost-of-living adjustments is also available. Please consult with your Pillar Benefit Specialist for pricing.
Can my partner participate in the program? Yes. Discounted policies are also available for spouses and domestic partners. Their policies will be billed directly to them at their home. Please consult with your Pillar Benefit Specialist for pricing.
Can I take my policy with me if I leave Perkins Coie? Yes, the policy is portable at the discounted premium. You will be directly billed for the coverage once you leave Perkins Coie.
